The following instructions will help you submit a claim form to start earning points:

 

1.   Download an Award Points Claim form.

2.   Complete all sections of the form, attach clear copies of sales invoices showing date of sale.

Make sure you include the ESAB part number, serial number, sales price and description of product sold. Be sure to print clearly.

 

3.    Print, sign and submit your Award Points claim form, supporting sales invoices and documents to:

Please allow 4 to 6 weeks for processing.

Once you earn 25 rewards points, you will be assigned an account number to access the online catalog www.awardhq.com .You will also receive periodic printed statements showing your account activity and balance.

 

Terms: Valid only for 2008 new (not reconditioned) equipment. Claim forms must be submitted within 90 days of sale. Forms without supporting sales invoices or missing information will be rejected. Serial Numbers are required for Plasma, MIG/TIG/Stick products. Award Points program may not be combined with any other offer or promotion.
